SPDX License Identifiers are a more compact / standardized
way of representing file license information.
See: https://spdx.dev/resources/use/#identifiers
This was done with the `ambr` search and replace tool.

Arbitrarily split up to make git bisect easier.
These unnecessary #include's were found by combining an automated tool (which
determined likely candidates) and some brain power (which decided whether
the #include is also semantically superfluous).

Problem:
- Output of decode and encode grow as the decode and encode
happen. This is inefficient because a large size will require many
reallocations.
- `const` qualifiers are missing on variables which are not intended
to change.
Solution:
- Since the size of the decoded or encoded message is known prior to
starting, calculate the size and set the output to that size
immediately. All appends will not incur the reallocation overhead.
- Add `const` qualifiers to show intent.

That makes the interface symmetric with decode_base64 and it's
what all current callers want (except for one, which is buggy).

The test cases are taken from RFC 4648.

I renamed check-license-headers.sh to check-style.sh and expanded it so
that it now also checks for the presence of "#pragma once" in .h files.
It also checks the presence of a (single) blank line above and below the
"#pragma once" line.
I also added "#pragma once" to all the files that need it: even the ones
we are not check.
I also added/removed blank lines in order to make the script not fail.
I also ran clang-format on the files I modified.
