The DMAContext is a simple pointer to an AddressSpace that is now always
already available. Make everyone hold the address space directly,
and clean up the DMA API to use the AddressSpace directly.

The old-style IOMMU lets you check whether an access is valid in a
given DMAContext. There is no equivalent for AddressSpace in the
memory API, implement it with a lookup of the dispatch tree.
