* Move USB to the top of the list, easiest for most people
* Split out the USB paragraph ready for inclusion for non-x86
* Mention the slight differences between BIOS and UEFI boot menu and
interactions (adding extra keycap entries needed)

- Remove outdated reference to booting from floppy disks in the

This run is in the english version, other languages will follow.

This commit basically creates the AMD64 variant as a copy of the i386 manual.
Further commits will be needed to differentiate the AMD64 variant from i386 where needed.
Commit includes changes for all translations.
